As Rabbi [name] mentioned on Shabbos, the Halachic Living Will is a simple and effective way to ensure that your wishes are followed in any medical situation, and that decisions are made in accordance with halachah.

Please download the attached halachic living will form. Then follow these steps:

1. Read the form and make sure you understand it. Any questions you have about the form will be answered at the program.

2. The form asks you to choose a healthcare proxy and a rabbi, as well as alternates, in case those people are not available. You are not required to choose an alternate for either role.
Before the program, it is worth considering who you want these people to be. In virtually all cases, these are simple decisions. Your healthcare proxy is the person you would want to make medical decisions on your behalf, if you are unable to do so yourself.
When choosing a proxy, issues to consider include the following:
a. the person’s ability to understand medical information,
b. the person’s ability to make decisions,
c. the person’s proximity to your location,
d. the person’s willingness and ability to devote the time and energy necessary to perform this job
Since there is always a chance your preferred proxy will be unavailable, it would be wise to choose an alternate proxy.
Discuss your decision with your proxies.

3. Decide who the designated rabbi will be. Rabbi [name] is available to do this for anyone. His contact information is: [contact info].
Alternatively, you can enter the name of any rabbi. Make sure to have a conversation with whichever rabbi you choose. You do not need to have this conversation before you designate the rabbi.

4. Fill out the form, BUT DO NOT SIGN IT.

5. Bring it, along with your driver’s license or any other form of government-issued ID, to the Open House at [location] on [date] at [time]. We will have witnesses [and a notary] to execute the documents.
Although it is best to fill out the form in advance, we will have blank forms on hand at the program as well.

6. Once you have had your form properly signed and witnessed at the Open House, you will receive an EMES card, provided by NASCK. The EMES card is a brief summary of the halachic living will that also ensures that your choice of healthcare proxy and rabbi can easily be determined in case of emergency.

2. Bring your UNSIGNED forms to the Open House
