Be sure to attend our Halachic Living Will Workshop, tomorrow, [date] at [time], at [location]!

The program will be quick and straightforward. The plan is to give a step-by-step tutorial on filling out the halachic living will form, and then to sign it in front of witnesses, as is legally required.

Please download the attached halachic living will form.

There are two options:

  1. You can come to the program with your form already filled out, BUT NOT SIGNED, and sign it in front of witnesses.
  2. You can fill out the form during the tutorial, and then sign it in front of witnesses. We will have plenty of pens and blank forms.

Remember, if you fill out the form ahead of time, DO NOT SIGN IT until you are in front of the witnesses. In order to be legally binding, the form must be signed in front of witnesses.

We also be giving everyone who executes their halachic living will an EMES Card, which is a version of your halachic living will that attaches to your driver’s license.

After you have your halachic living will form witnessed, you will receive an EMES Card, provided by NASCK. The EMES card is a brief summary of the halachic living will that also ensures that your choice of healthcare proxy and rabbi can easily be determined in case of emergency.
